Immunization
Upon arriving to the clinic please check in with the front desk.
Patients receiving an Immunization are advised to remain in the clinic 30 minutes following an injection in case of allergic reaction. This also applies to parents of children who are under 18 years of age who have received an injection. Parents cannot leave their children unattended, in case of a possible emergent treatment requiring parental permission.
Flu Shots
Wednesdays and Thursdays from 8 to 11:30 a.m.
Beneficiaries aged 10 years and up must present their Department of Defense identification card. Children 17 years and under must be accompanied by a parent or legal guardian.
Save time and visit https://munson.tricare.mil/Patient-Resources/Forms to download, print and fill out your prescreening paperwork -- DHA Form 116 Pediatric and Adult Influenza Screening and Immunization Documentation -- ahead of time. Only the signed page, printed on 8.5x11 paper, is required.
Active Duty Service Members
Active duty and Guard/Reserve members are required to get the flu vaccine. For tracking purposes, we recommend getting vaccinated through a unit, installation, or MTF vaccination event. The flu vaccine is typically administered by Munson in the fall, for maximum coverage. If you receive a flu shot outside the Military Health System, please obtain the information below and provide to Munson's Department of Public Health for entry in your medical record:
- The date the vaccine was given
- The vaccine name or code
- Manufacturer
- Lot number
